One REACH Theory of Change

REACH is a program of Evergreen Treatment Services focused on helping people who are experiencing or vulnerable to homelessness. REACH provides services through four domains: Outreach, Housing, Integrated Care, and Community Justice. While these domains have been core to REACH’s service delivery, REACH formally organized its work around these domains within the last five years. Consequently, REACH sought assistance in crafting a cohesive, forward-looking strategy that supports and enhances the work each domain delivers while demonstrating a unified neighborhood-based “One REACH” model.

Seva partnered with REACH to create and visualize the One REACH theory of change to describe the actions and goals of each domain and its relationship to their overarching goals. Through a literature review of materials from REACH, surveys, and participatory workshops, we supported REACH in developing a theory of change that describes REACH’s key strategies, the meaningful improvements that result from their work, their value towards the larger network of community support, and the cumulative impact of REACH’s work.
